The Brave New World of Patient Engagement

The empowerment and engagement of patients and families in the design and coordination of health care services at the levels of policy, organization and direct delivery has become a centerpiece of current healthcare improvement efforts. Guidelines and legislation supporting, and in some instances mandating, explicit and meaningful involvement of patients and families is accelerating internationally, through efforts of organizations including the World Health Organization, Planetree International, and many regional and local healthcare quality organizations. Efforts such as the CMS Partnership for Patients, Patient Centered Medical Home (PCMH), and the Patient Centered Outcomes Research Institute (PCORI) have pioneered new practical approaches to improving and measuring patient experience. In addition, consumer healthcare trends and innovations in personalized technologies are creating a brave new world in which patients and their families are becoming more involved in creating healthcare of the future. This session will highlight how these trends are improving safety, quality and the patient experience of care.

Consumer Trends in an Age of Empowerment

The patient and family engagement movement has empowered individuals as captains of their own healthcare teams. Patients, in turn, are looking to their healthcare providers to accommodate their preferences and address their demands for greater cost transparency, convenience and access.

This program challenges attendees to consider whether their healthcare organization is poised to meet these needs of increasingly informed and engaged consumers. The session explores the challenges inherent in transitioning from a business to business model to a consumer model of delivering healthcare.

Business As Usual No Longer an Option

Improved patient outcomes and better health care value are the driving forces behind health care reform efforts. Through a review of current literature, outcomes data and organizational case studies, this presentation provides an in-depth look at how person-centered care is a fundamental strategy for achieving these aims. Indeed, no longer can person-centered care be considering something that is “nice” to do. It is now nothing less than a quality and business imperative. This presentation will present the evidence substantiating a fair return on the investment of time, energy and financial resources to implement person-centered care in terms of improvements in a number of clinical and operational benefits. These include patient and staff satisfaction, patient safety outcomes, cost of care, and length of stay. With an emphasis on their impact on the bottom-line, specific topics covered include strategies to engage patients and families in shared decision-making, partnering with community stakeholders to improve population health and driving measurable improvements in the patient and staff experience.

Show Me the Evidence! The Evidence Base for Patient and Family Engaged Care

The case for patient and family engagement (PFE) has been built largely on anecdotal evidence, documented professional practice, and small scale tests of change. More comprehensive evaluative studies of PFE have been hindered by the sporadic nature of current activity and the range and complexity of the issues. But this is no longer the case. The case for patient and family engagement has never been stronger. There is now compelling scientific evidence that delivering healthcare within a culture of patient and family engagement creates better results: better health, better care, better culture and lower costs. But, too often, family caregivers are relegated to the role of bystanders once a patient is hospitalized. This talk examines the role that family plays on the healthcare team, and dissects the missed opportunities for delivering high quality, high value care when families are onlookers versus active participants in care planning, coordination, and management.
