Mission-driven Organizations Align to Make Person-Centered Dementia Care the Norm

FALLS CHURCH, VA. – November 5, 2013 – Three organizations with person-centered care at the core of their missions (CCAL – Advancing Person-Centered Living, Eden Alternative, and Planetree) today announced they have joined together as a leadership team to form the Dementia Action Alliance.  The Dementia Action Alliance will coalesce and connect the voices of people living with dementia, their care partners and other advocates to inform key policy, research, and service provider stakeholders about the importance and benefits of making person-centered care the standard for dementia care in our country.

Derby, Conn., June 28, 2013–VA New Jersey Health Care System’s East Orange Campus has been awarded Planetree Bronze Recognition for Meaningful Progress in Patient-Centered CareThe VA New Jersey Health Care System is the first health care organization to be awarded Bronze-level recognition since Planetree first introduced the recognition level last year.  It is also the first VA Medical Center nationwide to achieve recognition for its patient-centeredness, further establishing itself as a national leader in delivering Veteran-centered care.

December 5, 2012—In 2012 six new hospitals have been named Designated Planetree Patient-Centered Hospitals: Hospital Israelita Albert Einstein (São Paulo, Brazil), Sharp Memorial Hospital (San Diego, California), Barton County Memorial Hospital (Lamar, Missouri), Elmhurst Memorial Hospital (Elmhurst, Illinois), Stamford Hospital (Stamford, Connecticut), and Spectrum Health Reed City (Reed City, Michigan). In addition, three sites in The Netherlands achieved Designation: two long-term care communities, Woonzorgcentrum Archipel (Zorggroep Almere) and De Markt, Rivas as well as Gezondheidscentrum Notekraker (Zorggroep Almere) which is the first primary care center in the world to achieve Designation.

Planetree Honors Extraordinary Achievement in Patient-Centered Care During 2012 Spirit of Planetree Awards

October 24, 2012—Planetree has honored extraordinary achievements in patient-centered care at its 2012 Spirit of Planetree Awards and is pleased to announce that Picker Institute Executive Director Lucile O. Hanscom has received the 2012 Patient-Centered Care Visionary Leadership Award.  “Lucile has spent the past 11 years of her career dedicated to fostering a broader understanding of the theoretical and practical changes needed to facilitate a more patient-centered approach to healthcare, with a focus on the concerns of patients and other healthcare consumers. Global Patient-Centered Care Advocates, Planetree and Picker Institute, Partner to Create Harvey Picker Center of Innovation and Applied Research Program in Long-Term Care

September 4, 2012—Planetree and the Picker Institute share a remarkably similar vision for a healthcare system that integrates the patient perspective into every aspect of care, and the two organizations have a long-standing history of collaboration. The latest partnership endeavor is the formation of the Harvey Picker Center of Innovation and Applied Research and Program in Long-Term Care, to be developed through matching funds contributed by Planetree combined with funding from the Picker Institute. The goal is to develop an international hub of research and education with a core focus on identifying and disseminating innovative practices in person-centered long -term care which comes at a crucial time when the demands of health care reform require rapid deployment of innovative, person-centered practices. A vital priority for the Center will be to assist leaders with translating evidence-based knowledge to practice in support of sustainable practice improvement for residents, family, and staff across long-term care settings.

Building upon these core concepts, today Planetree is a collaborative, global community of health care organizations, innovating new solutions in the best interest of patients as well as caregivers.  Many of these innovations will be shared during dynamic, interactive workshops at the 2012 Planetree Annual Conference, September 30-October 3, in Palm Desert, California, at the J.W. Marriott.

Each year the Planetree Annual Conference brings together caregivers across the care continuum—CEOs, physicians, long-term care specialists, academics, patient advocates, nurses, executives, Veterans Affairs administrators, and many others—committed to the implementation of patient- and resident-centered approaches that activate patients and families to engage in their care plans. Planetree Announces National Designation Program Pilot for Continuing Care Communities

Derby, Conn, January 24, 2012.— Planetree has launched a multi-year pilot to begin the process of testing its Planetree Designation criteria to reflect the priorities of those who live and work in continuing care settings. Designation is aligned with empirical evidence supporting person-centered care as well as the reported experiences and preferences of residents/patients, families and caregivers. With the support of the Commonwealth Fund, a collaborative of organizations, including My Innerview, the IDEAS Institute, the Brown University Center for Gerontology and Health Care Research, and Planetree developed the Designation program’s criteria and measurement tools to assess performance of person-centered excellence. Literature review led to identification of gaps, resulting in the creation of additional Designation criteria.

Health Care Organizations Worldwide Honored with Planetree Designation

Derby, Conn., November 21, 2011—Three new U.S. hospitals–Carolinas Medical Center-Mercy (North Carolina), Kadlec Regional Medical Center (Washington), New York Presbyterian-Westchester (New York)– have been named Designated Planetree Patient-Centered Hospitals. New York Presbyterian is the first behavioral health center to be designated. Delnor Hospital (Illinois), Mid-Columbia Medical Center (Oregon) and Griffin Hospital (Connecticut) were re-designated this year since designation is granted for a three-year team and then re-assessed.

Derby, Conn., May 10, 2011—The Department of Veterans Affairs (VA) has selected Planetree to collaborate with the new VA Office of Patient Centered Care and Cultural Transformation in the development of the VA’s own patient-centered care model for Veterans who receive health care services at VA’s more than 1,000 points of care across the nation. Derby, Conn., February 16, 2011—Planetree, in partnership with Picker Institute, recently published the Long-Term Care Improvement Guide, a free resource available to download as a PDF. Now the Guide has its own website, www.residentcenteredcare.org. The Guide is a first of its kind compendium of innovations and approaches for transforming the culture of long-term care communities to be more resident-directed and relationship-centered. The featured practices and processes for organizational change span all models of culture change, (including Planetree, Eden Alternative, Wellspring and Green House) and apply across the long-term care continuum. The website is easy to navigate and showcases the Guide chapter by chapter, allowing users to pinpoint specific information. Users can also access a variety of practical tools such as sample policies, job descriptions, and evaluation tools utilized by continuing care communities that are effectively implementing culture change.

 

Health Care Organizations Worldwide Honored with Planetree Designation During 2010 Planetree Annual Conference

Derby, Conn., October 28, 2010—Longmont United Hospital (Colorado) and Flevozikienhuis (The Netherlands) have been named Planetree Designated Patient-Centered Hospitals. Designation is awarded for a three year term. Fauquier Hospital (Virginia), Northern Westchester Hospital (New York), Sharp Coronado Hospital (California) and Valley View Hospital (Colorado) were awarded re-designation this year in recognition of sustained excellence in the delivery of patient-centered care. Planetree offers free resources with practical approaches to patient-centered care
Derby, Conn.,September 17, 2010—Patient-Centered Care Awareness Month began four years ago in an effort to heighten awareness of patient-centered care among patients, families, caregivers, media, elected officials and the general public. Now with the passage of sweeping health care reform and CMS publicly reporting hospitals’ performance on patient experience via HCAHPS, the term “patient-centered” is firmly planted in the health care lexicon and is being used to describe everything from Congressional bills to MRI equipment. The term patient-centered was coined by Planetree more than 30 years ago to describe health care delivery around the needs of the patient; partnering with patients and families, encouraging involvement and personalizing care.

 

Long-Term Care Leadership Summit to Convene National Leaders to Deliberate About Practical Approaches for Sustained Culture Change
Derby, Conn.—Planetree is hosting a Long-Term Care Leadership Summit on October 5, 2010, with the aim of getting to the heart of culture change through an exploration of specific strategies for initiating and sustaining change by those on the front lines of the culture change movement. Delnor and Griffin Hospitals Named Designated Planetree Patient-Centered Hospitals
