Learn About Our Journey to Patient- and Family-Centered Care

Join us on the Sweet Home Chicago tour of Delnor Hospital, a 164-bed facility located in the western suburbs of Chicago. Experience our journey to patient- and family-centered care and meet the caregivers, physicians and Patient Partnership Council members who embraced the Planetree concepts and made them happen.

You will marvel at our recent 64-bed expansion project, due to open in June 2008. Planetree principles were incorporated into the design of these two beautiful units. You will experience the benefit of natural lighting, outdoor patio seating, patient and family areas with a full kitchen on every floor. Next you will tour Delnor’s Center for Breast Health, where you will find out why women in the western suburbs choose to have their mammograms completed at this all-digital state-of-the-art center.

We will share with you some of our greatest achievements on our journey which include: patient access to their medical record; Heartmath™; Dog Rx; music therapy; noise reduction campaign; development of the Patient Partnership Council; Cookie Program; Medical Advisory Team; Care Partners program; Destination Planetree; and Employee Spa Days.

Midwest Medical Center

If you could design and build a hospital that was 100% Planetree, it would be Midwest Medical Center in Galena, Illinois

Midwest Medical Center was the first Planetree Affiliate in Illinois and, on December 5, 2007, became the first Replacement Critical Access Hospital to open its doors in the state. Every detail of this magnificent facility incorporates the Planetree tenets. Upon entering the front doors you are immediately taken in by the warm colors, rich woods, soothing sconce lighting, vaulted ceilings and life size windows that point to the blue, Midwestern sky. From the water feature that keeps children entertained while giving parents a needed diversion, to the bubbling, copper fountain in the healing garden, you immediately feel that you are in a place where you will be well cared for.
The mahogany baby grand player piano provides mood-elevating audio, while the majestic ceramic-tiled staircase that leads to the family room encourages visitors and staff to climb instead of riding the elevators. An Incredible Tour of an Amazing Planetree Hospital

As Chicago’s first and only Planetree affiliate, Swedish Covenant Hospital is proud and excited to welcome Planetree Conference participants to tour our hospital facility. Featured on the tour will be our Intensive Care Unit, our remodeled Cafeteria and Art Collection, Mother Baby Unit, the Galter Life Center (Chicago’s premier medical-fitness facility), our new Emergency Department, and Swedish Covenant Hospital’s award winning Cardiac Rehab Center. Following the tour, a reception will be held in Swedish Covenant Hospital’s Healing Garden (designed by renowned landscape architect Maria Smithburg) featuring a buffet of Chicago-style foods.

Transportation to Swedish Covenant Hospital will feature a ride up Lake Shore Drive, a scenic highway offering views of Lake Michigan and the Lakefront area. There will be some outside walking during the tours, so participants should dress accordingly. Large golf-size umbrellas will be on hand in the case of inclement weather.
