Planetree partners with providers, large and small, across the continuum of care, guiding them along a path that enables caregivers to transform the health care experience they provide. For many providers, bringing Planetree’s concepts to life requires a shift not merely in operations, but fundamentally in their culture.
Planetree provides on-site expertise and coaching along with a variety of educational resources, including webinars, teleseminars and a database of materials including policies, procedures, and how-to manuals. Through technology and special events Planetree connects caregivers with a collaborative network of like-minded individuals sharing practical, field-tested solutions.
The implementation process focuses on inspiring caregivers and it begins with informational sessions and an organization assessment including focus groups with an array of stakeholders. The results–themes and priority focus areas–are shared with the board, staff and physicians, followed by the development of staff action teams and training, including leadership and staff retreats. Planetree provides guidance, expertise and resources to share practical strategies with the teams as they work toward their many goals, including patient-directed visitation, care partner programs, and open medical records.
