The Planetree Annual Conference is the longest running international conference on patient-centered care.  In 2013 our conference will be held October 6th – 9th in Montreal, Canada

Applying for a Passport

You can apply for a passport at many Post Offices™ around the country. At some locations, we’ll even take your passport photos for an additional fee. Use our PO Locator tool to find a nearby Post Office that accepts passport applications. Select “Passports” from the drop down under Location Types.

Information to keep in mind:

For new passport applications, you should bring…

1. Your Completed Application

You can complete it online through the State Department’s web site, or print and complete it by hand. New applicants, renewals, name changes or corrections, and lost or stolen passports each require a different application.

2. Two Types of Identification, with Copies of Each

You’ll need one proving U.S. citizenship…

  • Previously issued, undamaged U.S. Passport
  • Certified birth certificate issued by the city, county, or state
  • Consular Report of Birth Abroad or Birth Certificate
  • Naturalization Certificate
  • Certificate of Citizenship

 … and one current I.D.

  • Previously issued, undamaged U.S. passport
  • Naturalization Certificate
  • Valid driver’s license
  • Current Government ID (city, state, or federal)
  • Current Military ID (military and dependents)

3. One Passport Photo

Your photo should be in color and taken within the past 6 months. It should be 2″ x 2″ with a full-face, front view (head should measure between 1″ and 1 3/8″), and a plain white or off-white background. For an additional fee, some Post Offices can take your photo for you.
